    To A Troop, 501st Assault Helicopter Company, South Vietnam.

    14 October 1966

    Damaged.
    While enroute during a combat mission over South Vietnam, UH-1C 66-0497 was in level Flight at 1000 feet and 80 knots when it took 4 hits from enemy 7.62MM rounds. These impacts damaged the tail rotor, main rotor systems and the airframe. The helicopter continued flight and accomplished all mission objectives.

    Transferred to 71st Assault Helicopter Company, 501st Aviation Battalion, South Vietnam.

    20 November 1966

    Damaged.
    During a recon mission for Armed Escort over South Vietnam, UH-1C 66-0497 was attacking at 500 feet and UNK knots when it took 1 hit from an enemy 7.62MM round in the cockpit. This hit damaged the airframe. The helicopter continued flight and accomplished all mission objectives.

    5 July 1967

    Damaged.
    During a recon mission for Armed Escort over South Vietnam, UH-1C 66-0497 was in level flight at 800 feet and 85 knots when it took 1 hit from an enemy 7.62MM round. The helicopter was hit in the forward area, damaging the main rotor system. The helicopter continued flight and accomplished all mission objectives. It recovered to Chu Lai, South Vietnam.

    29 September 1967

    Damaged.
    During a recon mission for Armed Escort over South Vietnam, UH-1C 66-0497 was attacking at 300 feet and 80 knots when it took 1 hit from an enemy 7.62MM round. The helicopter was hit in the forward area, damaging the main rotor system. The helicopter continued flight and accomplished all mission objectives. Recovered at Tam Ky, South Vietnam.

    Damaged.
    During a Close Air Support mission over South Vietnam, UH-1C 66-0497 was in orbit at 1000 feet and 100 knots when it took 1 hit from an enemy 7.62MM round. The helicopter was hit in the Tail Section, damaging the airframe. The helicopter continued flight and accomplished all mission objectives.

    4 January 1968

    Damaged.
    During a Close Air Support mission over South Vietnam, UH-1C 66-0497 was in a descent at 800 feet and 80 knots when it took 3 hits from enemy 12.7MM rounds. The helicopter was hit in the Main Rotor Blade System, injuring a crewmember and damaging the main rotor blade system. The helicopter continued flight and accomplished all mission objectives.

    10 January 1968

    During a recon mission for Armed Escort over South Vietnam, UH-1C 66-0497 was attacking at 150 feet and 95 knots when it took 1 hit from an enemy 12.7MM round. The helicopter was hit in the tail section. The specific damage is unknown. The helicopter continued flight and accomplished all mission objectives.

    Damaged.
    During a recon mission for Armed Escort over South Vietnam, UH-1C 66-0497 was airborne at 50 feet and 60 knots when it took 2 hits from an enemy 7.62MM weapon. The helicopter was hit in the Main Rotor Blade system and tail, damaging both. The helicopter continued flight and accomplished all mission objectives.

    11 May 1968

    Damaged.
    During a Rescue and Recovery mission for Medical Evacuation in South Vietnam, UH-1C 66-0497 was attacking at 1200 feet and 80 knots when it took 1 hit from an enemy 7.62MM weapon in the underside. The helicopter continued flight and accomplished all mission objectives.

    6 June 1968

    Shot down.
    During a recon mission for Armed Escort over South Vietnam, UH-1C 66-0497 was attacking at 1000 feet and 80 knots when it took 1 hits from an enemy 7.62MM weapon. The helicopter was hit in the Engine Compartment, damaging the oil systems. The helicopter made an emergency landing and was later recovered by any means other than its own power. Both mission and flight capability were terminated.

    To U. S. Army Transportation Aeronautical Depot Maintenance Center (ARADMAC), NAS Corpus Christi, TX.

    From October 1968 to April 1969

    Transferred to Bell Helicopter, Fort Worth, TX.
    For conversion to UH-1M.

    From October 1968 to April 1969

    Converted to an UH-1M.

    May 1969

    Transferred to HHD, 44th Engineering Group, Korat RTAFB, Thailand.

    From June 1969 to October 1970

    Transferred to 240th Assault Helicopter Company, South Vietnam.

    29 October 1969

    Shot down.
    While in the target area in an unknown region, UH-1M 66-0497 was attacking at 400 feet and UNK knots when it took 6 hits from enemy ground fire from an unknown caliber. The helicopter made a forced landing. 66-0497 took off, fully flight capable. The aircraft was diverted or delayed after completing some mission objectives.

    November 1970

    Transferred to 56th Transportation Company, South Vietnam.

    December 1970

    Transferred to 166th Transportation Company, South Vietnam.

    January 1971

    Transferred to U. S. Army Aviation Systems Command (AVSCOM) Flight Detachment, 5th Army, Saint. Louis, MO.

    February 1971

    Transferred to U. S. Army Transportation Aeronautical Depot Maintenance Center (ARADMAC), NAS Corpus Christi, TX.

    August 1971

    Transferred to 8th Squadron (Air CAV.), 1st Army, Fort Knox, KY.

    From October 1971 to Circa 1994

    Transferred to Army National Guard.

    By October 1995

    To Boone Municipal, Boone, IA.
